Embark Behavioral Health is seeking a Health and Wellness Coach to join our team of mental health professionals. As a Health and Wellness Coach, you will work with preteens, teens, and young adults to develop and implement individualized health plans. You will support clients to achieve their physical and emotional health goals by providing education, guidance and support. In addition to coaching, the Health and Wellness Coach will also provide support to the clients families, aiming to create an environment of positive change.

Responsibilities
- Develop and implement individualized health plans
- Provide education and guidance on healthy lifestyle choices
- Encourage and motivate clients to achieve their physical and emotional health goals
- Collaborate with other professionals to provide holistic care to clients
- Document and maintain accurate records of clients' progress
- Provide support to clients' families in creating an environment of positive change
- Participate in team meetings and ongoing training programs
Requirements
- Bachelor's degree in a related field such as psychology, counseling or social work
- Certification as a Health and Wellness Coach
- Experience working with adolescents and young adults
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a team environment
- Knowledge of mental health issues and common treatment practices
- Familiarity with electronic health record systems.
