As a Behavioral Health Specialist, you will advocate and help assigned patients gain access to resources while confronting their medical and behavioral issues, provide patients with recommendations and behavioral interventions. Essential Functions: - Maintains a presence and active communication to the Primary Care Providers (PCPs) during clinic operating hours.
- Available for brief interventions by being in the clinic.
- Available for same day and scheduled initial interventions with patients.
- Performs brief, limited follow-up visits for selected patients. Whenever possible, refers patients to appropriate community mental health sites.
- May provide occasional intensive case management and traditional medical social work services such as referral management and coordination, acquiring durable medical goods, and patient advocacy.
- Provides a range of services including screening for common conditions, assessments, and interventions related to chronic disease management programs.
- Conducts risk assessments, as indicated.
- Assists in the development of clinical pathway programs, group medical appointments, classes, and behavior focused practice protocols.
- Provides brief behavioral and cognitive behavioral interventions for patients and families
- Triages patients with severe or high-risk behavioral problems to comprehensive behavioral health service (CBHS) or other community resources for specialty mental health (MH) services consistent with Step-up/Step-down criteria.
- Facilitates and oversees referrals to specialty MH/Substance Abuse (SA) services, and when appropriate, support a smooth transition from specialty MH/SA services to primary care.
- Educates PCPs in the basic principles of brief behavioral interventions, evidence-based therapy and cognitive behavioral interventions and reinforce their use in the medical visit.
- Provides assistance in capturing program evaluation and fidelity measures.
- Will be expected to see between 10-12 patients per day, in-brief (15 minute) interventions. May also provide 30-minute therapy sessions.
Requirements Education/Training - Masters degree in Social Work
- Active LCSW, LMHC, or LMFT licensure in the state of Indiana
- PH. D and APA certification (not required but highly preferred)
Experience - 1-2 years experience of post-degree clinical experience
- Additional primary-care experience (not required but highly preferred)
