NEW Grads Welcome!! CVHCare is looking for a qualified Occupational Therapist. Home health experience preferred not required. Assigned county Territory is Area # 1 (Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City ). Secondary area is likely to be Area 2 [Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont] OR Area 3 [Livermore, San Ramon, Sunol, Dublin, Pleasanton] Productivity: At least 30 points per week (points described below). Guaranteed Hour position: 36 hours. Traveler must complete at least 27 points in the week. Must be willing to travel outside the area or work weekends if required to maintain weekly visit expectations. If Traveler refuses patients, guarantee does not apply. Guarantee does not apply for 1st and last week of assignment, or for weeks in which there are holidays, or for weeks in which the traveler has time off. Non-guaranteed Hour position: There will be no productivity expectation. Can expect to receive between 24-30 visits (Points-Explained below) every week Other: Weekend coverage may be required. To secure a full caseload, Traveler may be required to go outside the service area occasionally. Traveler must be willing to see Covid+ and Covid Pui patients. Traveler must be willing to work weekends. Traveler must be willing to go outside territory, should the need arise. Payment made on per visit basis. See below for visit types and how they convert to points and how they convert to hours. This is how we will calculate how many hours are approved per day. - Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, Oasis Discharge, Oasis F/up = 1 point = 1.33 hours.
- OASIS ReCertification and OASIS RoC = 1.5 point = 2 hours.
Credentials: Current CA OT License, BLS, Drivers License, Auto ins, Auto Reg, TB (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug screen, Physical.
