Access Services is hiring a Lead Case Manager of Justice Related Services to serve adults with serious mental illness in the forensic system across Montgomery County, with an office based out of Plymouth Meeting(link removed)

The Lead Case Manager will provide training, oversight, and mentoring of program case managers, while carrying their own case management caseload. This person will be not only be interested in the systems of forensics and justice but also be a strong team player who is highly organized. As Lead Case Manager, services will be coordinated for adults being served using Blended Case Management guidelines. The Lead Case Manager will help ensure that the philosophy of care for Access Services is carried out, and that the service provided meets individual needs. In this role, support will be provided to individuals through the 5-intercepts of the forensic system by diverting incarceration, shortening incarceration and facilitating supported re-entry for adults with serious mental illness.

Essential Functions

  • Assist Director and Assistant Directors in the orientation of new case management staff, and in mentoring and training all case management staff.
  • Attend regular team meetings and case reviews.
  • Assist Director and Assistant Directors in community networking, relationship building, and in representing JRS with presentations made throughout the community.
  • Meet with new referrals to determine eligibility for case management program and complete preliminary intake paperwork.
  • Develop and write service plans in a timely manner with clear, measurable outcomes; complete all case notes and documentation; and maintain required billable hours within a workweek.
  • Engage JRS participants in jail, in the community, at court, in hospitals and through law enforcement, and provide case management services; facilitate support and linkage to program resources.
  • Attend county, committee and consumer meetings as needed, and facilitate multisystem meetings and provide support for complex cases.
  • Meet with program leadership regularly to provide and receive feedback on barriers to providing effective services in the community.
  • Participate in the on-call schedule and provide support to JRS participants as needed.
  • Complete all outcomes, statistical reports and billing documents as needed.
  • Comply with licensing regulations under Blended Case Management and participate in program audits.

Requirements

Requirements and Qualifications

Education:

  • Bachelors degree is required, preferably in a related field such as social work, human services, psychology and special education. Masters degree in a related field preferred.

Experience:

  • Completed case management training in advance, or upon hire with Access Services.
  • Six months of direct experience in case management with adults and/or children and families required.

Knowledge, Skills, and Abilities:

  • Excellent interpersonal, oral and written communication skills, and demonstrated ability to effectively work as part of a team.
  • Ability to effectively handle multiple priorities, remain organized and meet deadlines.
  • Ability to interact effectively with adults with mental illness, and skilled in suicide intervention.
  • Knowledge of BCM principles and regulations, the helping relationship, principles of wellness and recovery, and person-centered planning.
  • Knowledge of support principles of co-occurring population preferred. Knowledge of trauma informed care preferred.
  • Knowledge of criminal justice system, with a keen ability to exercise independent judgment within approved policies and procedures.
