Job Overview

The Care Coordinator will work in tandem with the healthcare facility staff and assigned clinicians to manage and maintain each patients healthcare plan including maintaining an accurate active patient list and scheduling patients at each facility as called for by the clinician. In addition, the Care Coordinator is responsible for training new staff on the care program and resources available. Incumbent will display a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service.

Responsibilities

(not limited to)

  • Ensure that all residents referred for care are seen and under care plans established by BH clinicians and that those care plans are being followed by facility staff over time.
  • Ensure that care plan adjustments that are made month to month are understood by facility staff and coordinate with clinicians as needed to further engage with staff to explain clinical strategies and directives.
  • Utilize scheduling tools to monitor and maintain care plan execution on frequency of follow ups and termination of care to update company databases on Active patient lists.
  • Organize orientations for new staff at client locations to explain the goals of the care program and resources, and to re-establish benefits for careful adherence to care plans for residents in the program.
  • Ensure that new facility admissions are being added to patient Active lists in a timely manner to keep clinician time slot availabilities full or alert Practice Managers of a need to add or contract clinical time slots building by building in order to address all patient ongoing care delivery needs.

Education, Experience, Training and Knowledge

  • Bachelors degree in a relevant field, additional relevant certifications a plus
  • Familiarity with behavioral health issues related to managing patient care delivery issues.
  • Knowledge of behavioral issues and social services strategies to alleviate problems. Knowledge of Units required.
  • Familiarity with Clinical processes and care plan adjustments from BH providers to facility staff, including medication changes.

Skills & Abilities

Exceptional communication skills, both written and verbal

Experience in healthcare, mental health field, preferred

Awareness of and commitment to all requirements, both legal and corporate

Solid computer skills, including Microsoft Office.

Sales Force knowledge preferred.

Superior organizational and interpersonal skills

Able to multitask and prioritize tasks

Able to act decisively and maintain discretion
