POSITION TITLE: Patient Navigator

REPORTS TO (TITLE): Director of Outpatient Services

DESCRIPTION OF POSITION:

The patient navigator serves as the main point of contact for the outpatient department, to help our clients navigate the program and broader mental health system, by offering referrals and supporting the client through the admission to discharge process. Collaborates with inpatient staff to provide continuity of care for all patients who are stepping down, and facilitates the intake process of clients from the community. Will communicate effectively with the clinical interdisciplinary teams, from inpatient and outpatient.

Key Responsibilities:

  • Facilitates step-down referrals from inpatient and from the community
  • Ensures completion of welcome packet by patients, prior to the commencement of group
  • Manages reports like the census, waitlist, and outpatient client tracker
  • Schedules psychiatric evaluations with the attending psychiatrist upon admission, and manages the calendar for follow-ups and exit interviews
  • Will perform clerical duties, such as checking in clients, sending outpatient emails for admissions and discharges, creating and breaking down charts, answering front desk calls, communication through email, note taking during staff meeting, etc.
  • Works with the business office to ensure there is a verification of insurance benefits prior to admission and follows protocols from intake to discharge
  • Will collaborate with the health information management (HIM) department to ensure discharged charts are sent in a timely manner.
  • Refers clients out to other services, as needed
  • Responsible for setting up transportation through Uber Health
  • Supports other staff members as needed
  • Effectively and promptly communicates problems, concerns, and safety issues to director of outpatient and interdisciplinary staff
  • Upholds ethics and customer service standards, while working with individuals from diverse cultural backgrounds
  • These duties are not intended to be all-inclusive

Requirements


Knowledge and Experience:

  • High School diploma or equivalent; Bachelors Degree in Psychology, Social Work, or closely related field of study preferred
  • One year of behavioral health experience and experience with severe and persistent mental health challenges
  • Current CPR certification
  • Familiarity with continuum of care and clinical terminology

Skills and Abilities:

  • Maintains confidentiality and professional boundaries of patients
  • Documentation meets current standards and policies and is done in a timely manner
  • Sensitivity to and willingness to interact with persons of various social, cultural, economic, and educational backgrounds
  • Proficiency with software and/or computer equipment (Microsoft Office applications including Outlook, Word, Excel, PowerPoint, and Dropbox)
  • Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines
  • Strong written and verbal communication skills
  • Ability to work independently and as part of a team
  • Good judgment, problem solving and decision-making skills
  • Demonstrated commitment to working collaboratively
  • Ability to work in a fast-paced, expanding organization

Physical Requirements:

While performing the duties of this job, this position is frequently required to do the following:

  • Use standard office equipment and access, input, and retrieve information from a computer. Use computer keyboard with manual and finger dexterity and wrist-finger speed sufficient to perform repetitive actions efficiently for extended periods of time
  • Communicate effectively in person or via telephone in a manner which can be understood by those with whom the person is speaking, including a diverse population
  • Give and follow verbal and written instructions with attention to detail and accuracy
  • Vision: see details of objects at close range
  • Coordinate multiple tasks simultaneously
  • Reach forward, up, down, and to the side
  • Sit or stand for minimum periods of one hour at a time and come and go from the work area repeatedly throughout the day
  • Lift up to ten (10) pounds
