At Eminence Home Care CT, we are a leading provider of in-home healthcare services, is seeking a Home Care Coordinator to oversee and manage our client care operations. As the Home Care Coordinator, you will be responsible for ensuring the delivery of high-quality and compassionate care to our clients, while also managing the schedules of our caregiving staff. You will work closely with our clients and their families, as well as our team of caregivers, to provide a seamless and efficient service that meets the unique needs of each individual.


Responsibilities

  • Coordinate and manage the day-to-day operations of our in-home care services
  • Conduct client assessments and develop personalized care plans that address their specific needs and preferences
  • Collaborate with caregivers to schedule visits and ensure all client care needs are met
  • Monitor and assess the quality of care provided by our staff
  • Oversee the recruitment, training, and onboarding of our caregivers
  • Communicate regularly with clients and their families to provide updates on care plans and address concerns
  • Document all client information and maintain accurate records

Requirements

  • Experience and/or working knowledge of Medicare.
  • Strong analytical and problem-solving skills, with the ability to think creatively and find innovative solutions to complex problems.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, their families, and other healthcare professionals.
  • 2+ years of experience in the home healthcare industry, with demonstrated leadership and management skills
  • Thorough understanding of client care needs, including health conditions and medications
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and their families
  • Strong organizational and problem-solving skills, with the ability to manage multiple tasks and priorities
  • Ability to work collaboratively with a team of caregivers and healthcare professionals
  • Proficiency in Microsoft Office and electronic medical records (EMR) systems.
