Requisition ID: req45141 Job Title: Virtual Supplemental Case Management (VSCM) Manager Sector: Social Work Employment Category: Fixed Term Employment Type: Full-Time Compensation: USD 54,900.00 - 61,000.00 Annually Location: New York, NY HQ USA Work Arrangement: Fully Remote Job Description Job Overview: The Preferred Communities (PC) Intensive Case Management (ICM) Program, funded by the Office of Refugee Resettlement (ORR) offers individualized case management to the most vulnerable IRC clients. In FY22 with additional funds from ORR, the IRCs ICM program created the Supplemental Case Management (SCM), a case management program that provides services to clients who exited their initial resettlement service period with additional need, but whose needs do not rise to the level of ICM eligibility. In FY23, the Virtual Supplemental Case Management program (VSCM) was created to provide services to clients in need of case management who are outside the serviceable area of an IRC office or other resettlement agency and help local offices who need additional virtual support. The Virtual Supplemental Case Management (VSCM) Manager will support the VSCM program by coordinating client enrollment process, tracking active clients, and assist with community outreach to provide trauma-informed, client-centered services and referrals to clients located across the 27 IRC resettlement sites as well as clients located outside of the IRC catchment areas. The VSCM Manager will collaborate with IRC staff to determine needs, coordinate community outreach, build effective relationships, and connect clients with community-based resources. The VSCM Manager reports to the Program Officer for Virtual Supplemental Case Management program in the Resettlement, Asylum, and Integration Department in the US. Major Responsibilities: Responsibilities include, but are not limited to: Support the management and implementation of the Virtual Supplemental Case Management program. This will include overseeing the referral and client enrollment process. Coordinate referrals and transfer requests from local IRC offices, the Virtual Resettlement Line (VRL), and other resettlement agencies. Serve as the initial contact to screen clients to determine eligibility for VSCM and conduct client intake assessments by phone or video conferencing with the use of telephonic or video interpretation as needed. Develop and maintain relationships with local offices and the Virtual Resettlement Line team to improve efficiency for referral and enrollment client process. Carry out a timely, professional enrollment process to ensure responsive, appropriate, and client-centered service delivery. Review VSCM program expectations, outcomes, and confidentiality with each potential client. In collaboration with the Program Officer, conduct meetings about potential cases or transfers and with local offices needing virtual support. Provide oversight of active caseload by tracking active clients and coordinate case assignments to Coordinators. Review client cases, assign to Coordinators and provide advice, direction, and support as needed. In coordination with the VSCM team, conduct community outreach, map community assets, build and maintain a network of state service providers and community partners as needed. Help maintain accurate and up-to-date electronic case files and required databases and consistently ensure the confidentiality of information relevant to cases. Work as part of a team to provide comprehensive strength-based services to IRC clients through case coordination and effective communication between VSCM Team. Other responsibilities as assigned. Job Requirements: Equivalent work experience or bachelors degree in social work or related field of study preferred. Work Experience: Minimum two years of experience working with refugees and/or immigrant populations and in domestic refugee resettlement or similar programs Relevant professional experience in coordination or management preferred Prior experience with program coordination strongly preferred Demonstrated Skills and Competencies: Proficiency in Microsoft Office applications and ability to use the internet and other digital tools for data entry, research, and problem-solving purposes. Ability to prioritize and manage time effectively Demonstrated ability to work effectively as a team member in a multi-cultural environment Excellent written and oral communications skills in English; proficiency in a language spoken by client populations a plus Ability to work both independently and collaboratively in a remote setting Self-starter with strong problem-solving skills combined with the proven ability to organize and multi-task Fluent in English, both spoken and written; proficiency in language(s) spoken by client group is desired. Ability to coordinate tasks and work through change in a proactive and positive manner.
