Ultimate Care NY, a leading home health care provider in the New York area, is seeking a Telehealth Nurse to join our team. As a Telehealth Nurse, you will be responsible for providing remote patient care through phone conferencing and monitoring patients from the comfort of their own homes. With your expertise and skills, you will help thousands of New Yorkers meet their tailored health care needs while promoting wellness and autonomy.

Responsibilities

• Conduct remote patient assessments, triage and provide high-quality patient care through phone conferencing.
• Ensure timely and accurate documentation of patient care and participate in telehealth care team meetings.
• Monitor patient conditions and provide appropriate interventions and referrals based on established protocols.
• Use telehealth technology to provide education to patients and their families in managing their care.
• Evaluate patient responses to telehealth interventions and adjust care plans as needed.
• Collaborate with other healthcare professionals and community resources to ensure continuity of care.
• Participate in quality improvement initiatives to enhance the telehealth program's effectiveness and efficiency.

Requirements

• Valid RN license in the state of New York.
• Bachelors degree in nursing (BSN) preferred.
• Minimum of 3 years of nursing experience, preferably in home health care or telehealth.
• Knowledge of telehealth technology and its application to patient care.
• Ability to provide high-quality patient care and maintain accurate documentation in a remote environment.
• Excellent communication and interpersonal skills to effectively interact with patients, caregivers, and healthcare professionals.
• Strong critical thinking, problem-solving, and decision-making skills.

Compensation: $70,000k-80,000k
