We are looking for a experienced Home Health RN Clinical Director to join our team in Phoenix(link removed) Must hold an active Arizona Registered Nurse license. The right candidate should be self-motivated, driven, out-going and efficient with their charting and review of documents.

Monday-Friday 8am-5pm mostly in office but some of the job will require patient visits and routinely keeping up with supervisory visits with our active field staff.

Requirements
At least 2+ years of Medicare Home Health experience required.
Excellent understanding of Home Care regulations, OASIS and PDGM.
Manage clinical Staff and direct reports.
Ensure compliance with all state / federal regulations.
Assist Administrator as needed.
- Case Opening Assessments
- Care plans
- Medication set up and reconciliation
- Wound care visits
- Supervisory Visits and patient visits
- Timely paperwork completion that meets DOH Compliance Standards
- Must be willing to travel within our main coverage area
- Positive team player who will also advocate for our clients and their needs - Make a Difference!
- Current AZ State RN License, TB test, CPR
- Clear Criminal Background Check and Drug Screen
- Detail oriented, multi-task, organized with good time management skills
- Reliable transportation, AZ Driver's License & Auto Insurance
- Experience with Wounds, Assessments, Medication set-up & Management
- Current and reliable cell phone and be easily reachable
