Position Overview:We are currently seeking a dedicated Case Management Specialist for a healthcare organization. This position is integral to providing continuous support and expertise in alignment with the organization's strategic plan. The Case Management Specialist will conduct comprehensive assessments, develop individualized plans, implement strategies, and evaluate patient needs. The primary objective is to enhance the quality of patient management and satisfaction, foster continuity of care, and optimize cost-effectiveness. This role encompasses responsibilities in case management, utilization review, and discharge planning, ensuring the well-being of all patients. Minimum Education: - Graduate of a Program in Nursing: Bachelor of Science level or higher is required.
- Master's Degree: Preferred.
- CCM or ACM Certification: Preferred.
Required Skills: - Proficiency in critical thinking, decisive judgment, and the ability to work independently with minimal supervision.
- Ability to thrive in a fast-paced and sometimes stressful environment, taking appropriate actions as needed.
- Excellent verbal, written, and interpersonal communication skills.
- Capacity to relate effectively to individuals of diverse age and demographic backgrounds.
- Strong analytical ability for data evaluation, making informed judgments, and providing recommendations regarding operations, budgets, protocol design, and report creation.
- Professional knowledge ensuring the quality of care across multidisciplinary service areas.
- Demonstrated leadership and organizational management skills.
- If not already Case Management certified, the candidate must be willing to obtain appropriate certification within 30 months of hire, given all other criteria are met.
Minimum Work Experience: - A minimum of five (5) years of management experience.
- Five (5) years of experience in utilization review and case management in an acute short-term hospital setting.
- Five (5) years of experience in discharge planning within an acute short-term hospital.
- Proficiency with Interqual or MCG criteria software.
- Advanced computer skills and proficiency in Microsoft Word, Excel, and PowerPoint.
- Preferred experience with MedHost EMR and MorCare.
Required Licenses: - Registered Nurse (RN) in the United States.
- Current RN licensure from the State of Arizona or a compact license accepted by the State of Arizona.
- Basic Life Support (BLS) Certification.
