POSITION TITLE: Director of Recuperative Care

REPORTS TO: Regional VP of Operations

LOCATION: Arleta, CA

CLASSIFICATION: Exempt

SALARY: $100,000.00 - $110,000.00

PERTINENT INFORMATION

  • Position title to start work January 2024
  • All positions require candidates to successfully pass a background check, LIVE scan and TB Test
  • Shift Schedule: Monday - Friday, 9A-5:30P (Flexible as needed to support program)

Director of Recuperative Care-Project Home Key Interim Housing

National Health Foundation Recuperative Care Program provides post-hospital care services to individuals experiencing homelessness who are transitioning out of acute care hospitals as well as skilled nursing facilities and community at large. The recuperative care program provides medical oversight in a trauma-informed and harm reduction environment that enables proper healing and recovery. Guests also receive comprehensive social services and mental health/illnesses support and connectivity in preparation for permanent housing placement. Responsibilities include developing and executing a quality assurance plan, maintaining accurate data utilizing a state-of-the-art database and ensuring legal compliance with contracts, communicate with stakeholders and over the program financial performance. In addition, the role includes acting as a public speaker, analyzing, and mitigating risks and problematic situations. The Director will create a highly engaged culture for both guests and employees and will maintain a strong, collaborative relationship with the community. They will provide leadership and the vision of the company.

Responsibilities and Initiatives

Success for the Director of Recuperative Care will look like:

  • Creating a highly engaged and robust program built upon radical hospitality and NHFs core values.
  • Ensure uninterrupted on-site 24/7/365 coverage of the facility.
  • Collaborates with Regional Vice President (RVP) to communicate staff expectations and best practices and performance appraisals. Work with RVP to build systems of change.
  • Maintain a structured, organized, and conducive work environment for all employees and community partners to feel safe and secure.
  • Effectively lead an interdisciplinary team including medical and social services to meet program and organizational goals including but not limited to budgeted census goals; Ensure accuracy of billing, and effective vendor management, etc.
  • Oversee guest quality of care services to ensure that the program meets or exceeds all recommended medical respite guidelines as established by the National Institute for the Medical Respite Care (NIMRC) standards, LAHSA standards and contractual obligations as well as NHF best practices.
  • Ensure the interdisciplinary team conducts, completes, and responds to all internal and external audits requests in detail. Must be able to complete Plan of corrective action (POCA) in accordance with LAHSA regulations and/findings.
  • Familiarity with LAHSA regulations and communications.
  • Collaborate with Regional Vice President to manage all financial performance of the program.
  • Must be able to read and comprehend all contractual obligations in tandem with the RVP.
  • Maintain agency records.
  • Maintaining accurate data utilizing a state-of-the-art database.
  • Establish and maintain relationships with community partners and agencies to strengthen services provided by the program.
  • Work closely with People and Culture to lead your team with integrity while establishing and maintaining a trusting, inclusive, disciplinary, and productive environment.
  • Uphold organizational policies, procedures, and standards and ensure legislative regulations are followed.
  • Participate in clinical consultation, program planning and supervision.
  • Complete and submit monthly reports to RVP.
  • Collaborate with Program Manager in ensuring smooth governing of the overall program to meet all areas of compliance.
  • Attending all training, workshops, and meetings as scheduled.
  • Provide training and in-services when needed to guide employees.
  • Develop, implement, and monitor day-to-day operational systems.
  • Develops personal growth opportunities, coaching & mentoring monthly.
  • To advise the Regional Vice President of Operations of pertinent information regarding the operations on a regular basis.
  • Model a positive professional image and adhere to organization dress code.
  • Flexibility to adhere to NHF schedule changes and on-call requirements to efficiently support the program. Must be able to provide coverage in the absence of the Program Manager and/or ready to substitute role.
  • To carry out all additional activities to meet the needs of the facility.

Requirements

  • Masters degree in mental health field (Social Work, MFT, or Psychology).
  • Licensed for 2 years and in good standing with California Board of Behavioral Sciences preferred.
  • Extensive experience working with homeless individuals with severe and persistent mental illness and co-occurring disorders.
  • At least 5 years of management/supervisory experience preferred.
  • Experience managing and providing oversight to multiple teams.
  • Highly detail oriented with excellent time management, organizational, written, verbal, and cross-cultural communication skills.
  • Passion for working with the population served. Experience working in interdisciplinary/multi-disciplinary fast paced environment (i.e., adult day care, clinic, residential, or similar type of facility) preferred.
  • Ability to cultivate an efficient and effective team and organizational culture.
  • Excellent communication skills both oral and written.
  • Positive and professional attitude.
  • Time management and strong organizational skills.
  • Computer proficiency in MS Office Suite. Ability to learn SharePoint and database software.
  • Must possess a valid California Drivers License, properly registered vehicle, and liability insurance.
  • Reliable transportation.
  • Bilingual in Spanish preferred.
  • TB testing required.
  • Fingerprint live scan background required.

Physical Requirements

  • The duties as described will require frequent standing and walking, as well as extended sitting. The employee will frequently bend/stoop, squat, reach above the shoulders, twist, and turn, kneel, and push/pull. The employee will frequently be required to lift/pull/push/move up to 50 pounds. Must utilize conversational speech for effective verbal communication. Noise level is high in some client care areas. Must be able to hear equipment alarms, overhead pages, and direct verbal communication.
