Position Summary The Case Manager (CM) will identify, screen, and assist individuals and families with accessing culturally affirming social support services. This person will work in collaboration with the Outreach Team and community partners to conduct compassionate outreach to hard-to-reach BIPOC residents who may be experiencing multiple barriers to care (i.e., housing, food insecurity, transportation, etc.) In the role, the CM will focus on the whole person and administered intake, assessments, strength-based case management, advocacy and community-based referrals, and linkages. Case Manager (Housing Specialist) works to increase housing opportunities for our program participants. The Housing Specialist/Case Manager will conduct outreach in the community to build relationships with landlords and provide housing search education in the form of 1:1 counseling, facilitating educational workshops, providing landlord/tenant mediation. The position requires candidate to work independently, and collaboratively as a team with the ultimate goal of increasing housing opportunities for the organization and placing program participants in housing that suits their needs. This position requires knowledge of Long Beach/Los Angeles (and surrounding areas) housing market trends, fair housing, landlord/tenant rights, and a strong understanding of the leasing process.
Essential Duties and Responsibilities The essential functions include, but are not limited to the following: - Provide ongoing support, crisis intervention and housing stability support and resource referrals,
- Provide additional support and assist as needed with landlord outreach, identification of rental vacancies,
- Assist with completion of rental applications and fee payment
- Conducting Client Intake by phone, zoom, or in person,
- Collect information about clients through assessments and screenings
- Make adequate referrals and/or linkages
- Communicate with external organizations to provide additional information and coordination to ensure the client obtains necessary resources
- Assist Clients with obtaining and completing forms
- Understands and works with the various support systems available in the community
- Identify emergency needs and make an approximate referral
- Serve as primary housing advocate and peer support person
- Maintain confidentiality of records relating to client's records
- Prepare and maintain notes in Unite s Platform and produce monthly reports
- Attend weekly/monthly staff meetings and participate in case conferences as needed
Requirements Minimum Qualifications (Knowledge, Skills, and Abilities) - Bachelor's degree in psychology, counseling, rehab counseling or closely related field
- A minimum of two years of related experience OR
- A Master's degree in social work, psychology, counseling, vocational rehabilitation, or a closely related field
- Ability to understand the characteristics of a wide range of intellectual disabilities
- Personal and professional experience with mindfulness or meditation work preferred
- Possesses excellent interpersonal skills and the ability to handle emotionally charged situations
- Ability to effectively communicate with external community partners, and clients verbally and in written correspondence
- Technical proficiency with Unite Us platform (Training Provided)
- CES/LAHSA experienced preferred
- Database experience (HMIS) strongly preferred
- Familiarity with TAY and HUD programs
