Overview:

THIS WORK MATTERS!

This specific position will not have a designated caseload but will provide case management services to children and adults with intellectual and developmental disabilities assigned to uncovered or vacant caseloads. This position may also provide coverage of specific tasks in instances where the caseload is covered or assigned and requires support as requested by Supervisors/Managers. The purpose of case management is to access, coordinate and assure the delivery of services and supports required by individuals with intellectual and developmental disabilities. Case Management will assist eligible individuals under their individualized plan in gaining access to needed medical, social, educational, and other services.

Temporary Assignments are not eligible for benefits.

As the Temporary Floater/Coverage Case Manager 2, other duties will include:

Perform complex case management duties for clients impacted by intellectual and developmental disabilities, including assessing, evaluating, and documenting eligibility for full spectrum Federal and State program services and benefits.

Provide coverage for a caseload of clients found eligible for DDSD case management services while the assigned Case Manager is on extended leave or the position is temporarily vacant.

Monitor services/supports via homes visits, service provider visits and phone calls to ensure that they are receiving appropriate financial, medical, training, educational, transportation, and other identified/agreed to services/supports

Report possible protective services issues and provide initial information to assist the investigation

Ensure that client health and safety issues are identified and resolved

Maintain case records and document actions taken in accordance with federal, state, local policy develop, submit, and modify necessary documentation to implement. change/adjust payments and program benefits

Develop, facilitate and coordinate resources necessary to provide consumers/families with needed services

Minimum Qualifications/Transferable Skills*:

Education/Training:

Bachelor's degree from an accredited college or university with major course work in social work, psychology/sociology, criminal justice or a related field. (Professional work at or near the level of this position can substitute year for year for the educational requirement.)

Three (3) years of increasingly responsible research, field investigatory or case management experience as a journey-level social services, court, criminal justice or corrections/protective services professional

Routine Telework: This position is designated as routine telework, meaning you will be working remotely (from home); a hybrid schedule also is available.
