Home Care LPN Qualifications:
- Active and in good standing Licensed Practical Nurse (LPN) license in the State of MN.
- Minimum 2 years of Licensed Practical Nurse (LPN) experience.
- Home Care experience preferred.
- Wound Care and Catheter experience (any bedside nursing experience) required.
- CPR Certification.
- Reliable transportation with a valid driver's license and auto insurance for travel within the service area.
Counties:
- North: Sherburne, Isanti, Chisago
- West: Wright, Carver
- South: Scott, Dakota
- East: Washington
- Mid: Anoka, Hennepin, Ramsey
Home Care LPN Schedule:
- Monday through Friday
- 8:00am to 5:00pm
- 1 weekend every 4 6 weekends, if needed
- Holiday rotation (approximately 2 Holidays per year)
- Approximately 6 patients a day.

Home Care LPN Responsibilities:
- Participate in ongoing interdisciplinary patient assessment.
- Collaborate in developing and evaluating the care plan with patients and caregivers.
- Notify the RN supervisor of any changes in the patient's condition.
- Provide physician-ordered services as per the care plan.
- Offer counseling and education to patients and families regarding healthcare needs.
- Maintain timely and accurate clinical notes to track patient progress.
