- Board Certified: REQUIRED
- Active OH License & DEA: HIGHLY Preferred (Willing to consider IMLC)
- BLS Certification: REQUIRED
- Flu Vaccine: REQUIRED at the time of credentialing
- COVID Vaccine: Preferred
- Preference to candidates available for ALL listed dates
