Homestead Hills is a leading healthcare facility dedicated to providing exceptional care to our residents. We are seeking a full-time MDS Coordinator (RN) to join our team.
Training Opportunity:
Homestead Hills is committed to investing in the development of our team members. We are open to training individuals who demonstrate a genuine interest in the MDS field and a passion for learning. We offer RNAC Certification after being employed for 6 months.
Responsibilities:
Conduct and coordinate the completion of the Minimum Data Set (MDS) assessments for residents.
Ensure accuracy and compliance will applicable regulations and guidelines.
Collaborate with interdisciplinary teams to develop comprehensive care plans.
Attention to detail and commitment to maintaining high-quality standards.
Provide education and support to nursing and clinical staff on the MDS process and requirements.
Qualifications:
Active RN License.
Skilled Nursing Facility experience.
Excellent organizational and communication skills.
Ability to work collaboratively in a team-oriented environment.
