HCA Solutions is looking for Pharmacist for inpatient, outpatient, and clinical pharmacy services at the VA Salt Lake City Healthcare System (SLCVAHCS) located at Veterans Affairs Medical Center(VA) is located at 500 S Foothill Blvd, Salt Lake City, UT 84148!

This contracted position includes the inpatient overnight shift with a schedule of seven (7) days on duty; then seven (7) days off duty. Outpatient operations are from 7am to 7pm Monday through Friday and Saturdays, Sundays and Holidays from 8am to 4pm. Inpatient Pharmacy operations are 24/7/365 coverage of all shifts.
Outpatient pharmacist functions consist of processing and dispensing of discharge and emergency room prescriptions. Outpatient contracted services will be located in the outpatient pharmacy and include services on several different shift schedules designed for coverage during the outpatient pharmacy hours of operation.
Be proficient in speaking, writing, and understand English.
Be proficient in communicating with patients and staff in a professional manner. This includes customer service skills, phone skills, and medical record documentation skills.
Be proficient in typing, keyboarding, and using office equipment.
Display a cooperative attitude and effectively work in a team dynamic to implement the mission and goals of the VA organization mission and goals.
Have the physical ability to lift, carry, and stock pharmacy supplies. This includes transport of supplies to wards, cache, or individual prescriptions to the dispensing window. Prescriptions may range from individual bags to cases of product or supplies up to 50 lbs. Must be able to stand, walk, climb stairs, and procure product from high or low shelving areas. Must be able to perform these duties while standing for their entire shift and maintain safe body mechanics especially while moving heavy items.
Have experience in outpatient, inpatient functions, and clinical functions (including medication reconciliation and residency preceptor abilities). This would include but not be to: limited proficiency in drug distribution and control, pharmaceutical care services, pharmacy practice management. Areas of emphasis include:
Drug Distribution
Prescription Verification and distribution.
Unit dose medication preparation, verification, and dispense.
Controlled substance dispensing.
USP 797 compliance for sterile compounding and IV room proficiency (IV admixture, TPN, Chemo, cytotoxic, or other compounding needs).
Discharge Counseling and Patient Counseling. Adapting counseling information to the understanding of the customer.
Pharmaceutical Care Services
Monitoring of drug therapy.
Providing Drug Information.
Medication reconciliation skills
Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas.
Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows up with patients on lab or test results to discuss the plan of therapy including changes in medication therapy, referral to primary care or specialty provider.
Adjusts medication regimens based upon pharmacokinetic and pharmacodynamics data.
Follows-Up with patients on lab or test results to discuss the plan of therapy, i.e., changes in medication therapy, monitoring, and additional testing requirements.
Monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues. Document findings per facility procedures. Actively participates in the VAADERS Adverse Drug Reaction (ADR) reporting system.
Gain or have experience and technical proficiency in computer software systems and automation. This includes VISTA, CPRS, ScriptPro, Talyst, Microsoft Office, and other programs used by the pharmacy.
Gain or have experience and technical proficiency in prepack label machine, automatic pill counting machine, unit dose packing sealer and machine, TPN mixer, hospital printers, and other auxiliary equipment.
Pharmacy continuous quality improvement activities and program management activities.
Adhere to VA codes of conduct such as dress code, use of electronic equipment, or other general expectations.

Be a US Citizen
Is a Pharmacy graduate from an American Council on Pharmaceutical Education (ACPE) approved college or university pharmacy degree program.
Have current and unrestricted Pharmacist and Pharmacist Controlled Substance license in good standing in a state, territory, commonwealth of the United State or the District of Columbia.
Have a National Provider Identification (NPI) number.
Have a NABP e-profile number.
Acute Care experience with PGY1 (Postgraduate year one of pharmacy residency training)
