HCA Solutions is currently seeking qualified candidates for a meaningful career caring for West Virginia's veterans as a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Certified Nursing Assistant (CNA) at the West Virginia Veterans Nursing Facility, One Freedom Way, Clarksburg, WV 26301.

THIS IS A PRN POSITION (MOSTLY NIGHT SHIFT) - YOU WILL BE ABLE TO CHOSE WHAT DAYS YOU'D LIKE TO WORK EACH HALF MONTH BUT THE OPTIONS ARE SPARSE.

Duties of the Registered Nurse (RN):

Assesses, identifies appropriate nursing diagnoses, plans, implements and evaluates patients care
Effectively advocates for ethical and holistic care by partnering in care planning to promote the autonomy, dignity, rights, values and beliefs of those we serve
Fosters a learning environment for patient/families, nursing, and other members of the healthcare team including students; facilitates formal and informal learning for patient/families, nursing, other members of the healthcare team, and community
Accountable for safety, identifies and corrects problems, and integrates evidence, and best practice into nursing care; uses data and evidence to improve patient outcomes. Aligns patient safety, self-safety and environmental safety as equally important for best outcomes
Engages in ongoing professional development; practices to the top of license within the legal parameters of the Nurse Practice Act, the Nursing Social Policy Statement, the ANA Code of Ethics for Nurses, and specialty standards
Leads and coordinates teams, appropriately delegates, coordinates care and collaborates with others as equal members of the inter-professional care team to integrate nursing knowledge
RN's could oversee the work of other RN's, LPN's and/or HSW's, as assigned.
RN's must participate in Interdisciplinary Care Plan Team Meetings to develop Individualized Care/Treatment Plans, direct consultations, receive and give recommendations to and from other disciplines to maximize care of residents.
RN's must administer medications as prescribed by treating Physician(s).
RN's must ensure timely documentation into resident's electronic medical records, per the policies and procedures and common practice of the facility, this will be discussed in extended detail during the facility orientation.
RN's must oversee all medical related emergencies.
RN's will provide for the emotional and physical comfort and safety of the residents.
RN's must respond to inquiries of family members, advocates and other interested parties, ensuring adherence to the State and Federal Confidentiality Laws, and the HIPAA Regulations.
Registered Nurse (RN) minimum qualifications:

RN's must hold a valid WV Registered Nurse License.
RN's must be licensed and are in good standing with the West Virginia Board of Nurses.
RN's must have a current Cardiopulmonary Resuscitation (CPR) Card.
RN's must possess a current and valid Food Handlers Card. All staff will be required to assist with meal pass.
Current PPD (Tuberculosis Skin Test), within the last year
Current Physical, within the last year
Duties of the Licensed Practical Nurse (LPN's) must be able to perform the duties under the LPN scope of practice such as:

Safely prepares and administers residents' medications under direction and authority of professional medical staff Applying dressings and bandages
Treating pressure sores and wounds
Monitoring residents and reporting changes
Giving medications quickly and efficiently in any emergencies
Caring for residents experiencing acute medical distress or who have a chronic medical illness
Monitoring food and liquid intake and output
Observes residents on a daily basis to identify, interpret, and document physical and emotional status and/or patterns and response to treatment or medication
Provides interventions to those individuals experiencing acute episodes
Participates in departmental or team meetings to discuss patient care and progress, shift activity and program policies and procedures
Monitors psychotropic as well as all medications given to residents and documents reactions; instructs residents on the purpose, dosage and side effects of medication
Obtains and records vital signs; assists physicians with exams; and maintains charts and logs, in particular the treatment reminder book
Acts as a team member for an assigned shift; plans, assists with the reviews of the work of other regular nursing staff such as Health Service Workers.
Carries out therapeutic and medical regimes
Sets up and maintains medical supplies and equipment

Required credentials for LPN positions:

Current, unencumbered WV LPN license
Current CPR certification
Current PPD from within the last year
Current physical from within the last year
CNA ESSENTIAL DUTIES AND RESPONSIBILITIES: (Other duties may be assigned):

CNA(s) must interact routinely with residents, observe behavior patterns and communicate significant observations to Charge RN and or Treatment Team. Document information regarding resident care and behavior as required in policy and procedure
CNA(s) must attend physical needs of the residents, such as assistance with personal hygiene tasks, and if necessary feeding and mobility
CNA(s) must run errands related to resident care and unit needs
CNA(s) must supervise patients going to and from treatment areas and in special care areas as assigned Participate in resident treatment planning and implementation under the direction of nursing staff and other professional staff involved in resident care
CNA(s) must participate in maintenance of a clean, orderly and safe environment; report needs for repairs and maintenance to appropriate staff
CNA(s) must assist in admission, discharge and transfer of residents as assigned to include personal inventory of all possessions and appropriate hand off communication
CNA(s) must obtain residents vital signs, height, weight and abdominal girth and document in resident record
CNA(s) must assist nursing staff in responding to emergency situations
CNA(s) must attend and participate in staff meetings and in-service training
CNA(s) must monitor and document nutritional intake as required
CNA(s) must maintain confidentiality in accordance with established policy and procedure and standards of care
CNA(s) must review all departmental policies and procedures to insure compliance
CNA(s) must possess the ability to be aware of others reactions and why residents react as they do Possesses the ability to tell when something is wrong or likely to be wrong; be attuned to noise levels and subtle changes in the therapeutic milieu
CNA(s) must monitor and document the monitoring of different resident observational levels, i.e. 15 minute checks, close constant observation or 1:1 observation of the resident with appropriate hand off communication
CNA(s) must have the skill to teach others in his unit how to do things whether it is an employee or a resident Requires the basic knowledge of age specific needs as related to adult, young adult or geriatric population
CNA(s) must complete mandatory training programs and annual training as required
