Hospice, Nurse Case Manager

In this role, you will care for, plan, provide, supervise, and document professional nursing care in accordance with physician orders and established policies and procedures. You will also get the chance to know your patients and their families and educate and counsel them while they or their loved one are on that road to recovery. A road that you are helping to pave.
Requirements:
- At least 1 year experience as RN
- At least 1 year of hospice nursing experience
- Must have an active RN license
