Daily Responsibilities For a RN Care Coordinator:
The Registered Nurse Care Coordinator is responsible for working closely with the patient/family and the Interdisciplinary Care Team to proactively assess, plan, implement, monitor and evaluate options and services to effect an appropriate, individualized plan for the patient across the continuum of care. The process addresses the patients multiple environments of care and healthcare system needs. The position requires customer service skills, ability to goal set and measure outcomes to effectively ensure the patient has the most optimal outcomes provided in the most cost-effective manner. Adheres to procedures and timelines as specified in the Managed Care Organization (MCO) contract and company procedures.
Requirements For This RN Care Coordinator position:
- Must be an RN in the state of SC with at least 2 yrs. of experience
- Good knowledge of personal computer and software for word processing and the ability to retrieve data from relevant computer systems.
- Self-starter with strong verbal and written communication skills.
- Highly organized with excellent follow-through skills.
- Comfort with change and quality improvement processes.
- Required to pursue ongoing education, certification, and/or self-development to remain current with industry standards and business objectives related to care coordination.
Hours For This Position
- Full Time M-F Day Shift: 8a 5p
- No on-call no weekends no holidays
