- Conduct comprehensive assessments of individuals seeking methadone treatment and or primary care services for new and assigned clients with opioid or alcohol addictions.
- Develop individualized treatment plans and goals in collaboration with clients
- Provide counseling and therapeutic support to individuals and their families
- Facilitate group therapy sessions and educational workshops on addiction and recovery
- Monitor and evaluate and document client progress and adjust treatment plans as needed
- Collaborate with medical professionals and other treatment providers to ensure comprehensive care for clients
- Maintain accurate and confidential client records and documentation
- Stay current on best practices and evidence-based approaches in addiction treatment and primary care
