Physical Therapist

Hospital at Home Program Duties:

• Deliver home-based physical therapy services to adult patients with general medical and orthopedic conditions.


• Consult with physicians and other health care workers, assuring satisfactory patient progress during different phases of rehabilitation


• Show true sympathy and compassion for optimal results.


• Drive to your patients homes throughout your assigned region on a daily basis


• Consult with the physician and an interdisciplinary team in the development of a plan of treatment

Requirements:

• PT with clear NY state license and a graduate from a CAPTE program


• Home Health experience preferred but not required


• Can use company UBER for patient visits


• At least 1 year of experience and trigger point therapy is a plus
