Regional Care Coordinator / Regional MDS Nurse

 

Our client provides post-acute services through their 30+ nursing facilities and many Home Health & Hospice agencies as well. This position requires an RN degree as well as Regional/Multi-site experience.  This position is at least 50% travel within TX and possibly LA/MN.  This position is essentially a Regional MDS Nurse. This person can be based in the DFW area, San Antonio or Houston, TX.

 

Essential Functions:

• Review and coordinate Medicaid and Medicare Case Mix data for the region

• Validate MDS coding, ICD-10 coding and RUG groupers

• Conduct detailed review of documentation to assist in accurate MDS 3.0 completion to ensure the appropriate level of reimbursement through accurate resident assessment

• Assess current processes and evaluate compliance with company standards, as well as Long Term Care regulations

• Provide education to licensed and non-licensed staff

• Identify opportunities and make recommendations as needed

Qualifications:

• Must have a current RN license

• 3 - 5 years MDS/RAI experience required

• Case Mix Management experience required in a long term care setting

• Must maintain current CPR certification

• Excellent verbal and written communication skills

• Excellent computer software skills

• Ability to multi-task and oversee multi-facility functions on a daily basis

• Ability to direct and lead staff of multiple facilities
