Essential Duties and Responsibilities:

• Provide progressive and meaningful face to face, telephonic, and written outreach and orientation services for each referred member to facilitate Health Home screening.


• Meet established goals and deadlines to include addressing identified gaps.


• Obtain applicable consent and enrollment forms when necessary.


• Schedule appointments with designated clinicians for each contacted member.


• Follow-up with each member to remind them of upcoming appointments and confirm completed appointments.


• Reschedule appointments if not completed.


• Maintain accurate case records and contact notes documenting all efforts within the designated time of attempted/ completed service delivery.


• Perform other duties as assigned.

Qualifications

• High School Diploma or equivalent required; some college preferred.


• Minimum 2 years of customer service and/or sales service.


• Computer proficiency and software skills required; CRM experience preferred.


• Access to reliable transportation.


• Strong organizational and communication skills.


• Bilingual preferred.
