Job Description:

• Under supervision of the Assistant Director or management designee for the Care Management team, the incumbent is responsible for coordinating transfers from the community into UCI Medical Center. These activities include coordination of M.D. to M.D. discussion; verification of benefits and authorization specific to the patient transfer. Uses several computer programs including Access, Microsoft Word, TDS, Invision to verify eligibility, obtain benefit information.

• Verifies eligibility; communicates/coordinates with medical staff.



 



Detailed Responsibilities:

• Work closely with a variety of healthcare team members (case manager, social worker, medical providers, insurance)

• Liaise with the multi-disciplinary team to facilitate accurate assessment of patient needs including reviewing the patients home set up to ensure appropriate provisions and care is in place.

• Communicate with the appropriate identified care agencies regarding patient discharge needs.

• Ensure that patients, relatives and carers are provided with information about discharge planning and that the information and timescales are fully explained and the appropriate level of support is provided. • Liaise with care/nursing homes to ensure timely assessments and completion of required documentation to facilitate discharge.

• Coordinate and work with the administration team for booking transport for patient discharge.

• Attend the regular discharge planning meetings and raise any relevant issues with the multi-disciplinary team to ensure resolution prior to discharge.

• Ensure that all relevant discharge information is recorded accurately, processed on time and is communicated to the multi-disciplinary team.

• Ensure awareness of the hospice discharge policies are communicated to all relevant parties. Keep abreast of developments in discharge practice in the NHS and community teams and support the inpatient team ensure that clinical policies and guidance are developed and up to date.

• Complete the correct Continuing Health Care (CHC) funding/ Social Service referral applications where necessary and follow this through.

• Performing initial intake to ensure proper client discharge from hospital with all appropriate needs for the continuation of care, entering and documenting all findings.

• Conducting morning, afternoon, and evening record review for all assigned clients ensuring there is no change in medical orders or conditions hindering hospital discharge.

• Linking clients to medical services when necessary (insurance, PCP, specialists, referrals)

• Coordinating home health visits and follow-up MD appointments.

• Entering client information into EPIC, Allscripts to maintain client records

• Performing administrative tasks such as answering phones and filing paperwork

• Collaborating with service providers to provide patient with necessary DME



Required Qualifications:

• Demonstrated ability to interpret health plan benefit packages and make determinations Demonstrated experience with referral authorization process Demonstrated knowledge of healthcare insurance EMR/LIS knowledge and experience

• Excellent written and verbal communication skills in English

• Experience in a hospital or medical office setting Experience using Microsoft Office Applications Demonstrated experience with referral authorization process Knowledge of ICD-9 and CPT codes

• Ability to establish and maintain effective working relationships across the HealthSystem

• Ability to maintain a work pace appropriate to the workload

• Must demonstrate customer service skills appropriate to the job

• Must demonstrate general understanding of medical terminology

• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties



Preferred Qualifications:

• Knowledge of University and medical center organizations, policies, procedures and forms



ESSENTIAL JOB FUNCTIONS:

• 20% - Accurately prepares, copies and distributes correspondence and documents.

• Composes, formats, proofreads and produces final copies of letters, documents, memoranda or basic reports from general instructions or information related to assigned responsibilities.

• 30% - Collaborates and effectively manages requests from case managers and social workers related to patient discharge and plan of care.

• Able to make appointments with providers for next level of care utilizing an understanding of insurance networks and discharge guidelines.

• 30% - Collects, compiles and assembles data from patient records.

• Maintains records of all discharges. Effectively enters and documents discharge information.

• Manages the compilation and distribution of IMD packets and LTC referrals.

• 20% - Effectively uses computer programs to manage discharge referrals, transition of care via the insured provider and patient preference.
