In joining our team, you will play a crucial role as a licensed Nurse Practitioner, leading the medical aspects of team-based care for individuals with serious mental illness. Responsibilities include conducting comprehensive clinical assessments, coordinating care, and providing support to our interdisciplinary team.
Responsibilities:
Conduct whole-person clinical assessments, gathering information from various sources and addressing both behavioral and physical health conditions. Lead the medical component of team-based care, providing guidance and support to firsthand Guides and Community Resources Guides. Support care coordination among medical and behavioral health providers, making individualized recommendations and referrals. Perform medication reconciliation post-discharge and communicate with individuals care providers. Develop and deliver educational programs to support nursing staff and affiliated medical professionals. Qualifications:
Empathy, compassion, and approachability Excellent communication and interpersonal skills Desire to support individuals on their path to wellness Comfort working as part of a multidisciplinary team and performing mixed roles Cultural humility, knowledge of trauma-informed care, and social determinants of health Familiarity with local providers and ability to maintain professional boundaries Intermediate computer skills, including familiarity with Microsoft Office Suite Valid drivers license, private auto insurance, and insurability under company coverage Experience:
5+ years clinical experience as a Nurse Practitioner, preferably in primary care Experience working with underserved populations and managing behavioral health and/or substance use disorder conditions Background in developing and delivering educational programs and care management Benefits:
