Candidate must be registered in the state of West Virginia

4.MANDATORY REQUIREMENTS:
4.1Mandatory Contract Services Requirements and Deliverables: Contract Services must meet or exceed the mandatory requirements listed below.
4.1.1Vendor will provide in-house occupational therapy services for the residents of Lakin Hospital. Vendor must include the following services:
4.1.1.1 Vendor must complete evaluation on all residents and all new admissions.
4.1.1.2 Vendor must be available for consultations at the request of the agency.
4.1.1.3 Vendor must document each resident visit and maintain medical and statistical records in accordance with Lakin Hospital policies and procedures.
