Responsibilities

Provide physical therapy services to SNF residents

Complete evaluations, treatments, and discharge planning

Maintain timely and accurate documentation

Collaborate with interdisciplinary care teams
Qualifications

Active North Carolina Physical Therapist license

Skilled Nursing Facility (SNF) experience preferred

Strong clinical and documentation skills
