Position Overview
Thrive Compassionate Counseling is seeking a dedicated and compassionate Senior Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity, serving patients in our Hospice Center located in beautiful Saint Petersburg, Florida. Work Environment

You will collaborate with our interdisciplinary team from the comfort of your home, utilizing advanced telehealth technologies and tools to deliver care effectively. The majority of the schedule will fall within regular business hours, with occasional evening and weekend shifts necessary to meet patient needs.
Key Responsibilities
- Conduct in-depth psychosocial assessments via interviews and observation, along with the analysis of medical records to identify emotional, social, and environmental factors affecting patients recovery.
- Engage in therapeutic counseling sessions with patients and their families, helping them navigate adjustment issues, grief, loss, and emotional well-being.
- Collaborate closely with the rehabilitation team to contribute to the formulation of individualized treatment plans, ensuring that psychosocial factors are integrated into each patients care.
- Assist patients in accessing vital community resources, such as support groups, vocational rehabilitation programs, and housing assistance, to facilitate a smoother transition back into the community.
- Support patients and their families in understanding available resources and post-rehabilitation services, promoting a comprehensive approach to care.
- Play a vital role in discharge planning, coordinating with patients to ensure successful transitions to home or other care settings, ultimately supporting their independence.
Patient Population / Client Focus
Your primary focus will be on a diverse patient population recovering from various conditions, including acute medical events such as strokes, spinal cord injuries, and orthopedic surgeries. You will have the opportunity to work closely with individuals who are navigating the psychological impacts of their health challenges, guiding them on their path to healing and reintegration into the community.
Qualifications
- A Masters degree in Social Work from an accredited program.
- Active and current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Florida.
- Strong communication skills, essential for building rapport with patients, families, and interdisciplinary team members.
- A thorough understanding of mental health issues, including adjustment disorders, grief counseling, and coping mechanisms.
Preferred Experience
- Previous experience working within a rehabilitation or medical environment is highly desired, as it will enhance your ability to address the unique needs of our patients.
- Familiarity with community resources and the healthcare landscape in Florida is advantageous for assisting patients effectively.
Compensation and Benefits
