The Assessment Coordinator provides professional, quality program services to the clients served by ReEntry House IRTS and works with other members of the team to assist clients in obtaining and maintaining an optimum quality of life consistent with client needs and preferences. The work uses evidence-based practices adapted for the agency and the client's specific needs. The emphasis is placed on working with clients in preparation for a discharge plan to a community setting and directly providing the services necessary to assist clients in achieving their goals.

Under the general supervision of the Treatment Director, this position functions as an integral part of the treatment team and is responsible for facilitating the admission process by communicating admission recommendations to the Treatment Director, Mental Health Therapists, RN, Practitioners and support staff. The Assessment Coordinator is also responsible for the completing Diagnostic Assessments and initial Functional Assessments for each client.

Duties and Responsibility:

  • Performs Diagnostic Assessments for all participants admitted to Community Foundations.
  • Completes initial Functional Assessments.
  • Coordinates the admission process for new residents including maintaining the waitlist.
  • Responsible for the initial recommendations for development of the Individuals Treatment Plan and Individual Vulnerable Adult Plan.
  • Helps defuse and de-escalate any potential behavior emergencies.
  • Identifies individuals prone to having behavior emergencies and develops additional programmingappropriate for their needs.
  • Support Treatment Director and Mental Health Therapists in clinical and administrative operations for vacations and absences including but not limited to providing clinical supervision during staff meetings, leading groups and meeting individually with clients.
  • Produces timely reports (i.e., Diagnostic Assessments, Functional Assessments requirements under MS 253B, etc.).
  • Participates in team meetings.
  • Provides individual therapy with residents, couple and/or family counseling when appropriate.
  • Upon successful completion of required medication test, administers medications according to Community Foundations policy and procedures under the supervision of the facilitys nurse.
  • Function as a team member responsible for surveying potential vulnerable adult concerns and other related clinical issues, and coordinates internal investigations and reports as needed.
  • Is familiar and maintain compliance with applicable Minnesota Statutes or rules, including, but not limited to DHS Rule 36V, MDH SLF (Class A), MS 253B, Maltreatment of Vulnerable Adults, Duty to Warn, Minnesota Data Practices Act.
  • Provides courteous and respectful care to all clients.
  • Works effectively and professionally with other treatment team members and with external providers and referral sources.
  • Utilize crisis assessment tools for suicidal and aggressive behavior.
  • On-call rotation for mental health crises and other emergencies including last minute shift coverage.
  • During scheduled rotation, with support from the Business Coordinator, secure coverage for any missed shifts, planned or unplanned, using all available agency resources. This may include on-site coverage for client care and per DHS requirements if no other coverage is secured.
  • Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
  • Maintain a pattern of regular work hours.
  • Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
  • Other duties as required.

Requirements

  • PreferredMasters or Ph.D. in Behavioral Science or related field,
  • 2,000 hours of experience providing services to individuals with mental illness,

Preffered

  • Licensed as a Mental Health Professional (LICSW, LP, LPCC, or LMFT)
